Admissions/MDS Nurse Position

We need an Admissions/MDS Nurse to join our team at Carespring Health Care Management in Loveland, OH.

$29.50 to $44 Per Hour

You'll assess new and existing patients to ensure that all of their care needs are addressed and documented on the MDS.

You'll Be Responsible For:

• Overseeing the MDS process following the RAI manual
• Working with other MDS nurses to complete initial assessments for new admissions
• Properly managing patient admissions scheduling, MDS assessment & care planning
• Completing required submissions weekly or as directed by the DON
• Reviewing validation report to ensure error-free submission
• Obtaining & reviewing QM's
• Working with IDT to improve resident outcomes
• Attending IDT Medicare & Medicaid meetings in collaboration with IDT to complete the MDS process
• Addressing the needs of residents by developing & updating comprehensive & nurse aide care plans
• Ensuring non-nursing sections of MDS & CAA's accurately reflect resident needs
• Monitoring care plans to identify resident needs
• Reviewing order recap reports & nurses' notes to ensure care plans are followed & determine whether additional OBRA MDS assessments are needed
• Monitoring residents' change of status & determining need for change per RAI
• Ensuring accuracy of supportive documentation according to MDS/RAI regulations
• Working with facility consultants to implement recommended changes
• Completing AIMS, Fall Risk scale, Braden, Progress note, Care Plan, nurse aide care plan (Kardex) paperwork

You'll Need To Have:

• A current LPN or RN license in the state of Ohio
• A flexible work schedule & the ability to work independently
• An understanding of the MDS process & a willingness to continually learn
• Knowledge in PDPM & RUG
