Job Details
Provide diabetes self-management education and training to individuals and groups.
Oversee planning, implementation, and evaluation of our DSME/T program.
Develop and direct program marketing, curriculum selection, and materials review.
Ensure DSME/T accreditation requirements are met and maintained.
Establish and maintain partnerships with community groups, payers, and referral sources.
Requirements:
Certified as a diabetes education specialist with a minimum of 3 years of experience.
1000 hours of practical experience and completion of a national certification exam.
Knowledgeable in chronic disease management, program management, and educational processes.
Proficient in computer skills, including Word, Excel, PowerPoint, and email.
Physically able to perform light physical activities, including standing, walking, and lifting up to 15 pounds.
