Tact Staff is seeking a travel nurse RN Case Manager, Acute Care Case Management for a travel nursing job in San Francisco, California.
Job Description & RequirementsJob Description: The RN Case Manager is responsible for Care Coordination, Care Transitions, and Discharge Planning throughout the acute care patient experience. Will be responsible for completing timely Initial Assessments, updated notes, and final notes. Will complete referrals to post acute levels of care. Will coordinate with the multidisciplinary team to ensure the discharge is on track. Requirements: - CA RN Licensure - BLS (AHA) - EPIC Experience - 1 year of experience with knowledge of post-acute levels such as Home Health, Hospice, Palliative Care, SNF, LTAC, B&C, Sub-Acute and Acute rehab Shift: 4 x 8 Days Fri-Mon, no on call required *Possible float to cover sick calls/PTO if census permits to Van Ness or Davies campuses*
