Description:
The Registered Nurse (RN) will provide skilled nursing care to patients of all ages in their homes, ensuring quality, safety, and continuity of care. This role requires coordination with the interdisciplinary team, the patient and family, and referring agencies while taking full responsibility for care management.

Essential Functions:

• Assess home health and/or hospice patients in stable and emergent situations to identify physical, psychosocial, and environmental needs (documented in clinical records, case conferences, and evaluations).
• Implement, develop, document, and evaluate the Plan of Care (POC) to ensure quality and continuity of patient care.
• Collect and incorporate data on physical, psychological, social, spiritual, and cultural factors that may influence patient/family health status into the POC.
• Maintain accurate, up-to-date patient records, ensuring all visits, contacts, problems, plans, actions, and goals are documented and updated as changes occur.
