The RN Care Manager at Providence PACE provides comprehensive nursing care management for elderly participants, including patient assessment, care planning, coordination, and ongoing evaluation to ensure appropriate medical and financial care. The role involves intensive medical management post-hospital discharge and close collaboration with interdisciplinary teams to support patient safety and well-being. This position requires a Washington Registered Nurse License and experience in long-term or geriatric nursing within a community-based setting focused on elder care.

$15,000 hiring bonus for eligible external candidates that meet required qualifications and conditions for payment.
The Registered Nurse Care Manager establishes a well-rounded care practice that includes: a patient assessment, care planning, implementation, coordination, and monitoring and ongoing routine evaluation that ensures medically and financially appropriate care. Responsibilities include: facilitating interdisciplinary team care coordination, intensive medical management of participants recently discharged from hospital and skilled nursing facilities. Additionally, this position will work closely with the IDT Social Worker and Physician to collect necessary medical and social information to ensure comprehensive decisions are made which ensure the safety and well being of participants.
